ALL students must be vaccinated to attend Washington State Public Schools. There are some personal and religious exemptions outlined in the forms below. Please note ALL exemptions MUST be signed by a medical provider.
Vaccines can be obtained at your local pharmacy or through Skagit County Health Department.

Please note there is an additional Tdap vaccine required prior to entering 7th grade!
Parents MUST send the requested information to the health room prior to the beginning of the school year. You can email it to either Monica Cook or Kelli Hutchens
