To Enroll your child you will need proof of your child's date of birth (birth certificate or passport), current immunization records and the forms in the boxes below.  You can choose between printing and filling them in or filling them out online and then printing and signing them.   You also have the option of coming by the office for an enrollment packet.

All students in Washington State can request a transfer from their resident district to another district.  Transfers are subject to the approval of the district a student wishes to transfer to.  If you wish to transfer to another district, please use the following application: Choice Transfer.  You can download the directions here.

If you reside outside of Conway School District you will need to complete the choice waiver application.  Applications can be found here: Choice Transfer Application.  You can download the directions here.

Para inscribir a su hijo, necesitará lo siguiente:

Comprobante de la fecha de nacimiento de su hijo (certificado de nacimiento o pasaporte)
Registro de vacunación actualizado

También necesitará documentación adicional. Puede pasar por la escuela para recoger un paquete de formularios o puede imprimir los formularios que se encuentran en el recuadro de la derecha.

Si reside fuera del Distrito Escolar de Conway, debe completar la Solicitud de Exención por Elección (Choice Waiver Application) que se encuentra en el siguiente enlace (en inglés); alternativamente, puede descargar el formulario impreso y entregarlo en la oficina.
